The obligatory procedure for submitting applications for maintenance grants, special grants for the disabled or aid payments involves filling in the application available in the Student Online Information System, i.e., Wirtualna Uczelnia (3rd-year students) or in the USOS system (in the case of the other years).  It does not, however, release students of the obligation to submit the original application and the required documentation in paper form.
